Heart Failure
Welcome to this course on Heart Failure (HF) where you will strengthen your understanding of how HF develops, how it is identified, and how pharmacists can meaningfully contribute to its management across diverse care settings. You will explore the key clinical features of HF, learn how to interpret essential diagnostic tests, and understand how different HF phenotypes influence therapeutic decisions.
Â
Across the modules, you will examine evidence-based lifestyle and pharmacological treatments, the impact of comorbidities on disease progression and medication choices, and the practical steps involved in assessing and managing acute decompensation. By linking clinical knowledge with real-world pharmacy practice, this course will equip you to recognise early deterioration, optimise guideline-directed therapy, support adherence, and contribute proactively to multidisciplinary HF care.
Â
From medication reviews in the community to monitoring treatment response in hospital settings, you will gain the confidence to apply HF principles safely and effectively, particularly where diagnostic resources or medicines may be limited.
Â
What you can expect in this course:
- Module 1 covers the fundamentals of HF, including key pathophysiology, common causes, and essential diagnostic tests. You will learn how to interpret results such as natriuretic peptide levels (NT-proBNP) and how to act appropriately on early signs and symptoms.
- Module 2 explores lifestyle modification and pharmacological management, focusing on the “four pillars” of HF therapy, medicines optimisation, treatment monitoring, patient counselling and strategies that empower patients to self-manage their condition effectively.
- Module 3 examines major cardiovascular and non-cardiovascular comorbidities, showing how they influence HF presentation, prognosis, and pharmacological therapy. You will learn how to tailor recommendations and anticipate medication-related challenges specific to multimorbid patients.
- Module 4 provides practical guidance on recognising and managing acute HF, including identifying triggers, interpreting key signs, symptoms and investigations, and supporting safe and timely escalation of care. The module also highlights the pharmacist’s role in preventing readmission and optimising therapy at discharge.
Learning outcomes
By the end of this course you will be able to:Â
- Understand the diagnosis of heart failure.
- Recommend the therapeutic management for a person living with heart failure.
- Use evidence-based strategies to manage heart failure in the context of common co-morbidities.
- Assess and support the management of acute heart failure, escalating care when needed.
Behaviour Change
The purpose of this course is not only to build your understanding of the diagnosis and management of HF but also to promote lasting changes in how you support people living with HF in your everyday pharmacy practice.
To support this, the course has been intentionally designed to go beyond passive learning. It will encourage you to reflect on your current practice, consider local challenges such as limited access to medicines or diagnostic tools, and strengthen your confidence to make evidence-based decisions that improve patient outcomes in your setting.
HF is a complex, multifaceted condition. As such, whilst optimised medication therapy is an integral part of treatment, it is prudent to move beyond this and adopt a comprehensive, patient-centered approach, including:
- Collaboration with the multidisciplinary team, both within the community and specialist healthcare settings, in effective care planning.
- Empowering the patient and caregivers to effectively manage their condition with lifestyle measures, self-care, and by ensuring they have the appropriate skills to understand when they need to refer to a healthcare professional due to changes in their condition.
While each module builds your knowledge and skills, they also include opportunities to reflect on your current approach, identify areas for change, and explore how to apply your learning in daily practice, even in resource-constrained environments.
We encourage you to engage fully with these behaviour-focused elements and use them to challenge yourself, support your colleagues, and provide consistent, patient-centred care that improves the lives of those affected by HF.
This course should take approximately 4 hours to complete.
